Data Request Service

Applicants could submit data requests with the following instructions:

  1. Applicants fill out the Data Request Form which can be accessed here.

- The information filled must be valid and complete
- Upload letter of recommendation signed by the authorized official (University/Institution)

  1. Applicants wait the response letter from the Head of PSTA for approximately seven working days. The response will be sent by email.
  2. Applicants are required to fill out and upload a statement letter.

  1. Applicants will be notified via email regarding the data submission schedule. If the applicant cannot attend without notice, the applicant is considered resigning and must re-submit the application. The same applicant cannot submit a data request if she/he has already submitted a request and has been served before.

Applicants can retrieve the data according to the data submission schedule and are required to fill out a community satisfaction survey here.
