Data Request Service
Applicants could submit data requests with the following instructions:
- Applicants fill out the Data Request Form which can be accessed here.
- The information filled must be valid and complete
- Upload letter of recommendation signed by the authorized official (University/Institution)
- Applicants wait the response letter from the Head of PSTA for approximately seven working days. The response will be sent by email.
- Applicants are required to fill out and upload a statement letter.
- Statement Letter template can be downloaded here.
- Upload the Statement Letter here.
- Applicants will be notified via email regarding the data submission schedule. If the applicant cannot attend without notice, the applicant is considered resigning and must re-submit the application. The same applicant cannot submit a data request if she/he has already submitted a request and has been served before.
Applicants can retrieve the data according to the data submission schedule and are required to fill out a community satisfaction survey here.
